What’s in your contract packet

You will find 2 or more agreements to sign, including:

• A GRIPA Provider Contract. This supplements but does not replace your present contract with GRIPA for HMO business.
• A ViaHealth PPO Provider Agreement. This will replace your present contract with ViaHealth PPO and cover all non-HMO business.

Each contract has four signature sheets. Please sign them all and return them to us. We will sign them and return one set to you for your records.

What you need to know about the contracts
