How GRIPA Connect Clinical Integration Works
getting connected in all the ways that matter
Clinical Integration delivers higher quality patient care by creating a connected community of physicians, hospitals, labs and imaging facilities with:
-
electronic access to complete patient information,
-
support from patient care managers, and
-
assistance to fulfill a commitment to evidence-based clinical care
The ease of electronic access
At the heart of our connected community is the GRIPA Connect Web Portal. Now operational, the web site can be found at: https://CI.GRIPAconnect.com. You can log on with a GRIPA-provided password. With a GRIPA-maintained central data repository at its core, the web portal allows patient information to flow freely and securely among physicians, hospitals, labs, pharmacists, etc. Read more about the web portal.
Help with patients
Case and disease management are critical to a Clinical Integration initiative. Fortunately, GRIPA has been offering these services to physicians for a number of years.
-
Nurses and social workers assist in case management for complex patients, reviewing charts and making recommendations to physicians and directly to patients. We have special programs for diabetic and elderly patients.
-
Our Clinical Pharmacy Program provides up-to-the-minute recommendations on such opportunities as half-tab doses and generic drugs, drug interactions and more.
If you are not already making use of our care management services, we urge you to begin by contacting GRIPA Care Management Services at 585-922-1520.
Improving patient outcomes
Clinical Care Guidelines are the final component of Clinical Integration. We are establishing these guidelines and publishing them on the web portal for easy access by all participants. Part of the requirement for Clinical Integration is that GRIPA must monitor and respond to non-compliance. Read more about Clinical Care Guidelines.
